Abstract We report an extremely rare case of primary aortocaval fistula with simultaneous development aortoenteric in a 68-year-old man. The patient developed under oral anticoagulation spontaneous intracaval aortic rupture. An emergency intervention was performed covering the aorto-uniiliac stent graft and femoro-femoral crossover bypass. One week later, transferred to our institution diagnosis psoas abscess suspected concomitant fistula. complete explantation endograft implanted it after extensive debridement aortobiiliac bypass, made bovine pericardium. postoperative course complicated, first by bleeding from left iliac anastomosis, then proximal anastomosis. entire explanted axillo-femoral bypass implanted. multi-organ failure died 3 months later. If possible, extended surgical resection all infected tissue situ reconstruction is gold standard. However, this therapy, there still high risk reinfection. Long-term antibiotic management mandatory.
